VSP Voucher Request Form

Please complete the following form if your club is within one or more of the following guidelines: 

  • The client is in an area where no Lions Club exists and there is no active District Task Force.

  • The Lions Club is out of funds.

  • There is no LEAP provider in the immediate area. Contact Katie Torgerson if you are interested in setting up a partnership with a local provider and using our LEAP Lab!

If any of the above guidelines apply to your club, please follow these steps to obtain a voucher for your client. Please be aware that providers who accept these vouchers are geographically limited. OLSHF will determine if there is a provider in your area who will accept the VSP voucher. 

  1. Vet that your client is eligible financially

  2. Submit an online VSP Application Request form (found on OLSHF.org/lions-hq *the password is md36)

  3. Follow up with your client to confirm they have redeemed the voucher

  4. Ensure client has completed the Client Survey. LCIF mandates the tracking and reporting of VSP stories, testimonials and photos.
